T-Ball Baseball-Softball Registration Form - 2017
Consent and Registration form must be signed, completed and returned before the deadline in order for your child/children to participate in the summer program(s). A late fee of $10 will be affixed on top of the registration fee. No registration will be accepted after March 24th 2017. No exceptions.
Return Forms to: School Office or City Hall!
Levels are determined by last grade completed. Please be aware due to limited diamonds and unforeseen circumstances some games and practices may be held out of town. T-ball will be provided New T-Shirts. Coach Pitch will be provided new jerseys. Minors and Majors can use last year’s jersey or opt for a new jersey.
